THIS Institute is carrying out an evaluation of the project, along with LCP Health Analytics, to gather evidence that will support its wider adoption and help shape future policies for effective proactive care.
The data leaflet below explains how Health Navigator (HN) uses your data to deliver the coaching. It also explains how how we invite patients and use their data to check that the coaching is working (Workstream 3).
